Symptoms after carpal canal operation: what to do?

At 15 days post-operative, you still feel pain, tingling or weakness in your hand? Don't worry, it's perfectly normal in most cases. The release of the median nerve takes time to produce its effects, and your body is still in an active healing phase.

Why operate a carpal canal?

The carpal channel refers to a narrow passage to the wrist that houses the flexor tendons and the median nerve, responsible for the sensitivity of the thumb, index, major and part of the ring.

When this nerve undergoes prolonged compression, several debilitating symptoms appear: nocturnal tingling, permanent pain, decreased grip strength and loss of sensitivity. The main causes include aging tendons, repetitive occupational activities, certain diseases such as diabetes, and hormonal changes related to pregnancy.

The operation consists of cutting the ligament that compresses the median nerve. Made under local anaesthesia in ambulatory, it lasts between 10 and 15 minutes. Two techniques exist: open-air surgery with a palmar incision of 4 to 5 cm, or the endoscopy requiring minimal opening.

What happens during the first 15 days after the operation?

The first two weeks represent the initial healing phase. Your surgeon applies a bulky dressing that you must keep without removing or wetting it until the check-in appointment.

The next day, you can use your hand for light gestures: feeding, washing, handling small objects. Swelling frequently occurs in the first few days. We recommend that you raise your hand regularly, especially at night by placing a cushion under your arm.

You must absolutely avoid any force effort, lift heavy loads or make sudden movements. Activities such as DIY, intensive gardening or carrying shopping bags are prohibited for at least six weeks.

What are the normal symptoms 15 days after surgery?

At this point, several sensations can worry you while they are part of the normal healing process.

Palmary pain are the most common symptom. You feel a marked sensitivity at the base of the palm, especially when supporting. This discomfort, called "pain of the pillars", results from the section of the ligament and gradually fades.

Persistent tingling do not mean failure. The median nerve, compressed for months or years, requires adjustment time. These paraesthesias gradually decrease but may last several weeks.

Electric shocks sometimes occur during certain movements. They testify to the ongoing nervous recovery and usually disappear themselves.

Weakness of grip prevents you from shaking strongly or leaning on your hand. This temporary decrease in force is absorbed in a few weeks to a few months.

The scar has a purple appearance, slightly swollen and hard to touch. It remains sensitive during contacts and its appearance gradually improves over the months.

What signs must be worrying at this stage of recovery?

Signs of infection represent an emergency: significant redness extending beyond the scar, purulent discharge, excessive heat or fever above 38°C. Check immediately if you notice these symptoms.

Algodystrophy is a rare but serious complication. It manifests itself by disproportionate intense pain, major swelling, progressive stiffness, excessive sweating and changes in skin colour. It requires specialized support.

Aggravation of symptoms must alert you. If your tingling increases rather than decreases, if the loss of sensitivity worsens, or if the pain becomes unbearable despite pain, contact your medical team.

What if pain or symptoms persist?

Respect scrupulously the prescribed analgesic treatment. Do not reduce doses prematurely, even if you feel better on certain days.

Maintain a gentle mobilization of your fingers. Make several times a day movements of opening and closing of the hand, without forcing. These exercises stimulate circulation and prevent stiffness.

In some cases, your doctor may request an electromyogram to accurately evaluate the recovery of the median nerve and identify any associated nerve compressions.

How long does complete healing last after an operation of the carpal canal?

Recovery occurs over several months and varies considerably depending on age, initial severity of compression and adherence to instructions.

The tingling usually fades between 3 and 6 months. The gripping force gradually recovers over 3 to 6 months. Recidivism remains exceptional, occurring in about 1% of cases after 10 years.

What gestures and habits help to recover better?

Raise your hand whenever possible during the first weeks. At night, place a cushion under your arm. This simple position considerably reduces oedema.

Gently engage your fingers As soon as possible. Perform 3 to 4 times a day series of 10 movements: fold and unfold each finger, form a complete fist and then open your hand.

Avoid vibrating tools for at least two months: drill, sander, mower. Vibration disrupts healing.

Respect the six-week rule No load. Your patience during this period largely conditions the long-term result.

Massage your scar from the third week if your surgeon allows you to do so. Use a neutral moisturizer with gentle circular movements.

When do you need to consult your surgeon?

Emergency contact If you notice signs of infection: fever, extensive redness, discharge, sudden pain.

Make an appointment quickly if you notice a gradual worsening of your symptoms after a period of improvement, or if a significant stiffness settles.

Report during next check persistent tingling beyond three months or an embarrassing weakness in everyday life.

What to remember at 15 days of operation

Two weeks after surgery, you are in the middle of healing. Palmary pain, residual tingling and weak grip are part of normal evolution.
